What is the typical income of a self-employed doctor in 2026?
The net annual income of a self-employed Belgian doctor varies widely with the specialty, agreement status and practice structure. GPs who have accepted the agreement earn around €70,000-110,000 in net annual income, specialists (cardiology, radiology, anaesthesia) between €120,000 and €220,000, and surgeons often more.
This spread directly shapes the strategy: a doctor earning €75,000 in their own name should maximise the social PLCI + NIHDI before thinking about a company. Above €120,000, an SRL/BV (doctors' company or standard SRL) opens up tax levers that typically save €8,000 to €18,000 a year.
NIHDI social status: your main lever
The NIHDI social status is a social benefit that the NIHDI grants to doctors (and other healthcare providers) who fully accept the national agreement between doctors and health insurance funds. In 2026, a doctor who has fully accepted the agreement receives around €5,685 a year (amount to be confirmed by publication of the official 2026 Royal Decree), which must be put into a supplementary pension agreement.
In practice, the NIHDI pays this sum directly to your social PLCI, IPC or group insurance provider. Not activating your NIHDI status = losing €5,685 net of pension savings every year. Over 30 years, that represents pension capital of around €250,000 to €350,000 depending on the net return.
Pension: social PLCI first, IPC through a company
The standard order for a self-employed Belgian doctor in 2026 is (1) social PLCI/VAPZ up to the ceiling, mostly funded by the NIHDI; (2) IPC through a company once you have an SRL/BV, capped by the 80% rule; (3) liquidation reserve for the cash left in the company.

Own name or doctors' company?
The usual switchover threshold for a doctor lies between €80,000 and €120,000 of net annual income. Above that, a company (standard SRL/BV or doctors' company) gives access to corporate income tax (20% SME rate or 25%), the IPC with the 80% rule and the liquidation reserve at an effective 18%, and lets you hold professional property.
The doctors' company (a specific form governed by the Royal Decree of 18 July 1972 and medical law) is generally preferred for groups of several doctors. A standard SRL/BV suits a doctor working alone or a family practice. The exact choice depends on your plans (succession, transfer, property).
Protection: suitable income protection and professional liability
Two types of cover are essential for a doctor: income protection insurance calibrated at 70-80% of net monthly income (30-day waiting period, up to statutory retirement age), and medical professional liability insurance with sufficient cover (often €5-10 million per claim depending on the specialty).
Check explicitly that back conditions, hand conditions and mental health conditions (burn-out) are included in your income protection, as they are often excluded from generic policies. For professional liability, watch out for "exploratory procedures" and "outside the facility" exclusions.
2026 action plan for a self-employed doctor
The order of actions to optimise your financial situation in 2026:
- Check that your NIHDI status is activated and properly allocated to a social PLCI or IPC, so you do not lose the €5,685/year.
- Maximise your social PLCI up to the ceiling (~€4,702 in 2026), see the PLCI guide.
- If your income is > €100,000-120,000, look into switching to a doctors' company / SRL with an IPC (80% rule) and a liquidation reserve.
- Review your income protection (70-80% net cover) and medical professional liability (€5-10 million).
- If you have a company car: fully electric only (combustion-engine cars have 0% deductibility since 2026, see company car).
